Discharge community link nurses employed by the scheme to facilitate discharge from hospital enabled 83 per cent of people who preferred to go home to do so.
The proportion of deaths at home among patients covered by the scheme rose from 17 to 42 per cent and deaths in hospital fell from 63 per cent to 45 per cent. In addition, the total costs of end-of-life care fell by 8 per cent.
National cancer director Mike Richards said the results were 'extremely exciting'.
